Tax Preparer Affiliate Client Payroll Summary #

This form is the Tax Preparer Affiliate Client Payroll Summary. Please use the form below to submit your Client List for Commission Pay.

How to use this form:

1. Enter your EFIN number on the top right of the form.
2. Enter your email you use with us. (You will enter your name at the end when you sign)
3. Enter the client info: Taxpayer Name, the last 4 of their SSN, the Tax Preparation Fees Charged.
4. Click on the signature line Tax Preparer Affiliate – you will then be prompted to sign the document and then enter your email. 

Once it has been submitted you will get an email verifying your submission. Please be sure to click the link in that email for verification. The payroll department will not receive your submissions if you do not do this and will delay your commission payout. Please email payroll@unlimitedtaxes.com for any other questions.
